Everyparticipant is required to complete parts A & B (no Part C).
Print and complete two (2) copies of forms A and B plus two (2) front/back copies of your health insurance card stapled to your forms. (This is required by BSA.)
All completed forms and documents will be turned in to your Patrol Leader in advance.
The SWC Healthy and Safety staff will not collect these forms – they will stay with your Patrol Leader at all times.
